Employee Enrollment List

INSTRUCTIONS:

1. Download the Enrollment List excel file below *(Do not alter the file in any way)
2. Enter in the information for all ACTIVE employees ONLY *(Do not have inactive employees on the list)
3. Save the excel file
4. Complete the information on the web page & submit the excel file

NOTE: Only the employees listed on the submitted excel sheet will be enrolled.
Any employees not listed will be removed from the pool. The list you submit will replace all previous enrollment lists.
